Overview
To appeal a decision from the Ontario Disability Support Program (ODSP) or Ontario Works (OW), you first must request an internal review of that decision from the office that made it. Learn more about what can be appealed and how to file an appeal on our Appeal and Hearing page.
There is no fee to file an appeal with the SBT.
Instructions to fill out a form
To view, print or email SBT forms, you need the Adobe Reader software. You can download this free software from Adobeâs website.
To download forms to your computer, right click on the title of the form and select âSave link asâ. You must open the form on your computer using Adobe. Do not open the forms in your internet browsers (e.g. Chrome, Safari, or Edge) as some browsers will not allow you to complete and save the form.
Most internet browsers will store (or “cache”) documents on your computer so you can access them faster in the future. This may mean you are not accessing the most up-to-date version of our forms. To clear your cache, while on the internet, hold down the âCtrlâ key and the âF5â key at the same time, or press Ctrl+Shift+Delete.

What to do if you canât find a form
The SBT does not have forms for some things, such as requesting an extension of interim assistance or asking for an adjournment. If you are having trouble locating a form, or you do not see a form listed for what you need to do, please contact the SBT.
