Forms, filing and fees

Find all Licence Appeal Tribunal – General Service (LAT-GS) forms and learn how to file applications or appeals and pay the filing fee.

Use the online payment portal to pay your filing fee and file your Notice of Appeal or Application by email.

Forms

LAT-GS forms are listed in two categories below:

  • Application and appeal forms
  • Other forms

For information about what form to file, see appeal and application types.

Before filing an application or appeal, you may want to get legal help.

Download forms to your computer by right clicking on the PDF icon and selecting “Save link as” and then open the form on your computer. Do not open the forms directly in your browser (e.g., Chrome, Safari) because some browsers will not allow you to complete and save the form.

The method of payment depends on how you are filing your appeal or application. If you are filing by email, the filing fee must be paid online using Visa Credit Card, Visa Debit, MasterCard Credit Card or Debit Mastercard. If you are filing by mail or courier, the filing fee can be paid by certified cheque or money order, payable to the Minister of Finance.
